Hero One Program Application

The Hero One Foundation Board of Directors reviews applications for Hero One grants during specific funding cycles.
​
Cycle 1 is to open January 31 to April 30 each calendar year. Cycle 2 is to open August 31 to November 30 each calendar year. Applications must be submitted by April 30 or November 30 at 5pm EST in order to be considered for funding.

The application is completed using the Hero One Program form and submitted via email to Vicki.Foley@HeroOneFoundation.org.

We will consider grants for the following:
1) requests for a product (i.e wheelchair, hand controls for a vehicle, etc).
2) requests for a service (i.e recovery training sessions, personal care hours, etc).
Please indicate which you are applying for on your application by checking appropriate box and providing details.

In order to submit your application, you will need to email the following documents as a part of the application process.
A.) W2 or other proof of full household income.
B.) Letter of need for the service being requested from either your training facility or care provider.
C.) Please include one photo of the person applying for a grant. This photo is for identification purposes only and will not be published without written consent of candidate.
PLEASE NOTE THE FOLLOWING IMPORTANT DETAILS
Candidates must possess a positive, can-do attitude to be considered for funding.
All applications must be received by posted deadline in order to be considered.
Incomplete applications will result in failure to be considered for funding.
Hero One Foundation retains the right to make the final determination on whether or not a grant will be awarded.
An application is NOT a guarantee of funding.
Hero One Foundation funds US residents only and must reside in the US.
Hero One Foundation does not fund medical bills or medical expenses.
The average Hero One grant awarded is between $500-$1,500.​
